Guitar makes bad buzzing when plugged into sound system

Hey! I have had this guitar for several years now and have had this problem off and on, but now it's really bad. Whenever I plug it into a sound system, it gives me feedback really bad and I can't make it stop. When I put my hand on the jack where it's plugged into my guitar, it gets quieter, and sometimes if I'm moving around, it'll come and go. It's almost acting like there's a bad connection or a loose wire or something. I've done everything I can, but nothing has helped. Also, my on-board tuner stopped working when the buzzing got really bad and I haven't been able to get it to work since. It worked fine up until the last time I plugged it into a system. The batteries are new and the light flashes, but it won't detect the strings being plucked. When I plugged a tuner into it using a 1/4 inch wire, the tuner would read the buzzing that my guitar is making. Any ideas what it could be? I'm praying it's the output jack and not the pickup or control box. Thanks!
